Have you or the patient had a temperature of 100.4 or greater in the past 24 hours?
Have you or the patient had any difficulty breathing in the past 10 days?
Do you or the patient have a cough, nasal congestion or sore throat that is different than your normal baseline and with symptoms that are not mild or infrequent?
Do you or the patient have a new loss of taste or smell?
Do you think you have had a potential exposure to someone that has confirmed COVID-19?
Have you been told to quarantine by any public health authority including DPS or TSA?
